I have read quite a bit of info over the last few days and particularly about treatments. Can some of you more experienced HS sufferers who have surgery or other treatment give any advice or share some experience good or bad with it in mind that I am to go and see my surgeon for the first time next week 12th nov.

It appears from reading that there are many differing professional views on the success of each treatment and maybe some personal experience may help me with my appointment and the way forward for me, it is always good to be armed with relevant information.

Hi John,

The question is what you mean when you say "surgery."

As far as I know, there are three main types of surgery used to "treat" HS. The first is a first aid treatment, the incision and drainage of a lesion ("lancing"). Depending on the size of the abscess and amount of buildup inside it, a tube may need to be placed for a bit.
The second is excision, where the surgeon removes an area of the skin/tissue and possibly a sinus tract.
The third, the biggest monster of them all, is skin grafting. This is reserved for the most severe cases of HS, as it is an involved procedure with relatively long recovery time.

I know nothing of the third option, but I can tell you my experience with the first two.
I had my first drainage at the tender age of 16, when I woke up in the morning to a lesion the size of a penny. I couldn't walk, and ended up in the emergency room later that day. Surgery was scheduled the following morning. With no complications, I was discharged a few days later.

A few months later, after all seemed well and it appeared the hole was healing (Mind, this was before I was diagnosed. The doctor simply told me I had to wear underwear that didn't chafe)... The hole didn't heal. I went to see a surgeon and he informed me that I needed to have an excision. He said he could perform it in a clinic with a local anesthetic. I was wary, but agreed.

I'd like to take a moment to go off on an anesthetic tangent. MAKE SURE YOU TALK TO YOUR DOCTOR ABOUT ANESTHESIA. I cannot stress this enough. Apparently, local anesthetics don't take well in areas of infection. Given the fact that even when a lesion is in remission there is still a risk of minor infection, this is very important. Granted, different people have different pain thresholds, but there should be no surprises on the operating table.

Moving on! I had the surgery (And I wont distress you with the details), and after a few months... it didn't heal. Went back to the surgeon, he effectively told me I would need to keep having surgery until they caught the little ****** ( Shocked Ed Bot!  Lips Sealed ), and that was all I needed. I was out the door.

I had two more similar experiences, but they were more of the same.

As for other treatments... What have you tried? Have you done anything alternative (i.e. not Western medicine)? Nothing works for everyone, but there has been success with homeopathy, reflexology, etc. I am currently keeping my HS in check with a regimen of dietary supplements and yoga. Yoga works well because in addition to being a form of exercise that people with HS can take part in (Because it's slow-paced and sans-sweating), it boosts the energy in your body and helps all your systems function better. Jus a thought.

I realized now that this post is about a month old. Sometimes I don't visit the forum so frequently. How did the meeting go? Keep us posted!

Yes just saying surgery is a little vague from me, sorry. It is easy to see afterwards isn't it.

I am at stage 3 in one area possible 2 and the general surgeon has referred me to a plastic surgeon with the idea of wide excision surgery removing the whole infected area. My appointment with the plastic surgeon is on 21st dec so I will post afterwards.

I realise you say you have no personal experience of the this option but thanks for your informative reply on the others. I am quite concerned about having this surgery to be honest as I have read the success is not always there for everyone. I have 3 areas of "attack" left groin (really really bad) lower abdominal area (really bad) right groin (bad, getting worse) from the initial surgeon appointment he seems to be more concerned with the left groin only at the time (that looks the worse but the abdominal area is often the most painful for me) my worry with the wide excision surgery here is that there is quite a large area of about 5 open sores which are constantly leaking and now the abscesses have moved to the right of this area and I have 2 large abscesses that now come up right beside / inside? my testicle. This weekend I have a new abscess that has come up in the same area but higher up so that is 3 areas to the right of this "bad area" so I am thinking the surgeon would have to take out some size area to get it all, if he was just to take out the really bad area that has been giving grief for years then I am just going to get the problem further over to the right......surely.... Does anyone have any thoughts on that.

The other 2 areas I suffer are getting worse ie my abdomen and right groin area. I have only ever had a flare up in 1 area at a time but right now I have a flare up pretty bad in both groins, the left one (new abscess) is about the size of golf ball right now and growing quickly it has probably doubled in last 24 hrs and I expect today / tonight to be the worst as it peaks. My right groin has had a flare up for 3 days now, very small in terms of abscess and again a different spot to the usual in that area and although small it is very very very painful in fact one of the worse I have had to be honest, it does not look or feel like much but is agony. My lower abdomen has built up over the weekend but that burst yesterday and that area is never too bad once it has burst but over the weekend and now I am being attacked on 3 fronts.....got an appointment to my GP in an hour, he cannot come to me and I am in going to be in absolute agony getting there but I need something for the pain and I need something to help me sleep as I am sleeping really poorly right now.

Avital I have not tried any other things to help me besides antibiotics etc I have had from my dr, I have only really been diagnosed about 1 month (by myself) before having it confirmed by the surgeon. My dr and the surgeon both told me a derm cannot help but I am going to insist I go to see one as I have other areas that the problem is not as bad for surgery (i think) so I have to try something. I have read about quite a few ideas people have and what has helped some but not others and I am going to seriously look into trying different things but wish to wait and see the plastic surgeon first in 2 weeks time and the dermatologist as quickly as I can.

Sorry for the tales of woe folks but it helps me to write and I am sure some of you guys out there relate to it, understand it and others may get some benefit from my ramblings helping to identify there problems etc.

I've had excision & wide debridement. I had to change the dressing on a LARGE GAPING wound on my inner thigh for 6 months strait! 1st surgery didnt work, had to have a second.

Only recently thru www.hs-foundation.org did I didcover a specialist in California who was able to give me relief from a stage 3 flare up within 24 hours!!!!!!!! Please look into Kenalog Injections if you are still suffering rather than scarring up your body with radical surgery
